Instructions:

1. Fill out the contact information section in its entirety. We will use this info only to update our records and contact you regarding the stock contest.
2. Pick up to 4 (four) stocks to assign your $100,000 virtual portfolio. Use the link to the right of each input box to search for the ticker symbol by company name. The ticker and company name will be automatically filled into this form when using the link to the right.
3. Assign each of your stock picks with an appropriate percentage of investment of the $100,000 portfolio. You do not have to invest each stock equally, but you must invest 100% of the $100,000 virtual portfolio.
4. Submit the form. You should receive an email confirmation that we've received your portfolio before the contest begins. If you have any questions or problems with this form, please contact our offices.
